Please call us at 847-674-3600 to set up your appointment. We hold office hours Monday through Friday from 10am to 6pm. We have Saturday and evening hours by appointment only.

Initial Evaluations: 45-60 minutes long. Please bring a list of all medications. This visit includes a full work-up by Dr. Shives with our extensive history and physical. All of the forms needed for this visit are available to download and fill out at home via our website.

Follow-up Visits: Approximately 30 minutes. No paperwork necessary.

Overnight Sleep Studies: We offer our patients sleep studies every night of the week. Patients normally arrive at 9pm and leave the center around 6:30am for their SCHEDULED sleep study. Arrival times can vary depending on your sleeping routine.
